Do you take insurance?We currently do not, however, most of our services cost the same as a copay to a specialty office or Physical Therapy. We can also print off a billing statement with the necessary codes for your treatments that you can submit to insurance for reimbursement.

What if I can’t fill out the paperwork online?Please come in 20 to 30 minutes before your appointment time to fill out the paperwork in the office, this will help so that we don't take time away from your appointment.

Where can I find the office?We are located in the Winnebago Plaza behind the First National Bank off of Elida Road. Our office is at the end of the hall on the right hand side.

How do I schedule an appointment?You can go to our website and click on the “Book Appointment” button, this will redirect you to a site where you can request an appointment time within our facility hours or you can call our office directly at 815-216-4286.

How long are the appointments?The first visit can take up to 1 hour, depending on if you filled out the paperwork before coming into the office. Any future appointments can range from 30 minutes to 1 hour and 15 minutes depending on the recommended treatments.

What should I wear to my appointment?We recommend comfortable clothes that aren’t too tight. If you are being seen for your knee we recommend wearing shorts or pants that are able to roll up past the knee to help with the examination and to receive proper treatment.

What should I bring with me to the appointment?Make sure you bring an ID, a medication list (if you are filling out paperwork in the office), and a form of payment for the appointment.

What type of payments do you accept?You can pay cash or with a card (credit, debit, or HSA), we also offer financing through Scratchpay.

How much is it to be seen? How much will future appointments be?The initial visit $49 due the day of, future appointment cost will depend on treatment packages or other recommended treatments for your care. Most costs are similar to copays at any Specialty Office or Physical Therapy and we do offer discounted packages.

What can I expect on my first visit?Filling out new patient forms (if not done online before the visit),then the Nurse Practitioner will conduct a series of exams and learn more about the condition you wish to seek treatment for. You may also receive treatment the day of the initial visit depending on the findings from the Nurse Practitioner.

What is a Nurse Practitioner?A nurse practitioner (NP) is an advanced practice registered nurse and a type of mid-level practitioner. NPs are trained to assess patient needs, order and interpret diagnostic and laboratory tests, diagnose disease, formulate and prescribe medications and treatment plans.
